OFFICIAL RULES
(Eventgroove Fundraising, or just “Eventgroove” ) works with Compassionate Home Health Care, Inc. (CHHC”), a U.S. 501(c)(3) tax-exempt public charity, which receives all donations made through the Eventgroove online platform (“Platform”). CHHC runs Rock the Rock Green Bay as well as Rock the Dock Green Bay 360 VR. We have community based programs that have affected thousands of people each year. These programs take time and money. We have the time, and hopefully you have the money to help support us. In return, we have come up with some pretty amazing prizes. Below is the small print we need to know to keep us all on the up and up.

SUBMISSION PRICE
The purchase price of a single submission is $50.00 each. A “purchase” shall not occur and be binding on CHHC until funds received for the same have cleared the bank. Credit cards are accepted. Submissions may be purchased online at https://rtd.eventgroovefundraising.com/rtd360 or form one of the many community partners throughout NE Wisconsin.

Each submission purchased shall constitute a “submission” into the Contest, and the person to whom the Contest submission is registered shall be the “Donor.” The winning Donor shall be the “winner” of the prize. Payment to or delivery of any prize will be made to the Donor. The winner must supply his/her social security number before the prize is tendered. It is the sole responsibility of the Donor to allocate a winning prize to other participants in case of a multiple submission registration, and the winning Donor shall hold harmless CHHC in case of conflicting claims or disputes among multiple claimants. By purchasing a Contest submission and choosing to participate in the Contest, each Contest participant acknowledges and agrees that CHHC has the right, in its sole discretion, to elect not to proceed with the Contest for any reason whatsoever or for no reason, and in such event, a participant’s sole and exclusive remedy shall be to receive a refund of the full price paid by such participant for each Contest submission purchased, without interest.

QUANTITY OF SUBMISSIONS
There are a maximum of 10,000 submissions offered for sale.

ELIGIBILITY
Purchasers of Contest submissions must be legal residents of the United States and 18 years of age or older on, or before, August 29th, 2020. The Donor must supply his/her social security number to receive a winning prize. If an Donor’s credit card is dishonored prior to the drawing, the submission will be declared invalid, until a replacement is received. All credit card charges found declined after the drawing must be honored. A $30.00 charge will be assessed for non-sufficient funds. Submissions will be electronically recorded for Internet purchasers upon order. Contest submission numbers will be automatically assigned in order of date purchased. Contest submissions are non-transferable and non-refundable; all submission sales are final.

INTERNET PURCHASES
This Contest will be posted on the Internet at https://rockthedockgreenbay.org/Contest/. It is the purchaser’s responsibility to provide a valid email  and  contact information in order to successfully receive the submission confirmation and to remove any system blocks that would prevent the successful delivery of the email confirmation.

TAXES AND IRS RULES
Winner is responsible for any and all federal, state and local income or excise taxes, fees, assessments and like charges associated with the prizes. Winner will be responsible for any or all delivery fees and/or are responsible for retrieval of their prize. The value of your winnings is considered taxable income. Winner must complete a W-9 form before receiving the prize. Please consult your financial advisor to discuss any impact on your personal taxable income. The IRS has taken the position that amounts paid for chances to participate in lotteries, Contests, or similar programs are not gifts and therefore, the price of submission does not qualify as a deductible charitable Contribution. DRAWING
The Contest will take place on August 29th, 2020 on or about 8:45 PM. Submissions will be available for purchase until the time of the drawing. The winner, if not present, will be notified within 7 business days. It is the responsibility of the purchaser to provide a valid, deliverable mailing address, phone and/or email address for notification. 

PRIZE LEVELS
3 levels of prices will be offered. The more submissions that sell, the better the prizes that are offered, up to and including the Tesla. 

LEVEL 1 – (Once 750 Submissions Sold) – VIP Rock the Dock Green Bay Experience, Apple 13” MacBook Pro, Oculus Quest All-In-One VR Gaming 128GB Headset, Apple iPhone 11 Pro Max 256GB, Samsung Galaxy S20+ Ultra 5G, Weber Gas Grill, Callaway Men’s Strada Golf Clubs, Apple 11 Inch iPad Pro 256GB, (5) $1000 Amazon Gift Cards, (5) $500 Amazon Gift Cards.
LEVEL 2 – (Once 2000 Submissions Sold) – Harley-Davidson Motorcycle & Bullfrog Spa become available.
LEVEL 3 – (Once 3500 Submissions Sold) – The Tesla Model 3 all-electric car becomes available.

PRIZES
The winner will receive one of the following prizes.

The Grand Prize is the winner’s choice between a (1) 2020 Tesla Model 3 Standard Range+  Approximate Retail Value (ARV) of $38,000 or $30,000 cash (except as stated below). The Grand Prize Winner must make an election in writing between the car and the cash option no later than 5:00 p.m. September 10th 2020. A minimum of 3500 submissions must be sold by August 29th, 2020 for the Grand Prize Drawing to take place. In the event less than 3500 Contest submissions are sold before the drawing, the Grand Prize will not be drawn.

The vehicle comes base model factory equipped and winner(s) are also solely responsible for any and all state or local license, title, registration, cost differential between the value of the car and the cash alternate prize, taxes, or fees associated with the vehicle, as well as insurance (proof of which must be shown prior to delivery) and pickup or delivery costs at the dealership as well as any non-standard options chosen by the winner and negotiated with the dealership. All winners of vehicles must make an election in writing between the vehicle and the alternative cash payment no later than 5:00 p.m. on the tenth business day after the drawing.  All vehicles are subject to availability at the automobile dealer selected by CHHC and may be substituted with a comparable vehicle by CHHC or with the cash alternate prize. The Car, if selected, will be transferred to the Grand Prize winner “as is, where is, and with all faults”. CHHC does not make or provide any representation, guarantee or warranty, expressed or implied, in connection with the Car and accepts no liability or responsibility regarding the construction or condition of the Car. CHHC does not warrant that the Car is of mercantile quality or that it can be used for any particular purpose.  No express warranties are given and no affirmation of CHHC by words and/or actions will constitute a warranty.

The 2nd Prize is the winner’s choice between a 2020 Harley Davidson Softail Slim APV $18,000 or $12,500 cash (except as stated below). The Prize Winner must make an election in writing between the motorcycle and the cash option no later than 5:00 p.m. September 10th 2020. A minimum of 2000 submissions must be sold by August 29th, (1) 2020 for the 2nd Prize Drawing to take place. In the event less than 2000 Contest submissions are sold before the drawing, the 2rd Prize will not be drawn. 

All winners of the motorcycle must make an election in writing between the motorcycle and the alternative cash payment no later than 5:00 p.m. on the tenth business day after the drawing.  All vehicles are subject to availability at the motorcycle dealer selected by CHHC and may be substituted with a comparable motorcycle by CHHC or with the cash alternate prize. The motorcycle, if selected, will be transferred to the 2nd Prize winner “as is, where is, and with all faults”. CHHC does not make or provide any representation, guarantee or warranty, expressed or implied, in connection with the Motorcycle and accepts no liability or responsibility regarding the construction or condition of the Motorcycle. CHHC does not warrant that the Motorcycle is of mercantile quality or that it can be used for any particular purpose.  No express warranties are given and no affirmation of CHHC by words and/or actions will constitute a warranty.

The 3rd Prize is the winner’s choice between a (1) Bullfrog Spa R7L APV $12,500 or $7,500 cash(except as stated below). The Prize Winner must make an election in writing between the Spa and the cash option no later than 5:00 p.m. September 10th 2020. A minimum of 2000 submissions must be sold by August 29th, 2020 for the 3rd Prize Drawing to take place. In the event less than 2000 Contest submissions are sold before the drawing, the 3rd Prize will not be drawn. 

All winners of the Spa must make an election in writing between the Spa and the alternative cash payment no later than 5:00 p.m. on the tenth business day after the drawing.  All Spas are subject to availability at the HotTub Medic in Appleton, WI selected by CHHC and may be substituted with a comparable Spa by CHHC or with the cash alternate prize. HotTub Media has a delivery area of 50 miles around zip code 54911. If the winner lives outside that area, the cash option will be invoked.

The 4th Prize is the winner’s choice between an All Expense Paid trip to Rock the Dock Green Bay 2021 in August 2021 “Green Bay Experience” APV $3,500 or $2,000 cash. The Prize Winner must make an election in writing between the Trip and the cash option no later than 5:00 p.m. September 10th 2020. There is no minimum submission sale amount for this prize to be active. Winner will be awarded (2) Two round trip airline submissions from the closest major airport to the Winner and Green Bay’s Austin Straubel Airport. Winner will be given a midsize rental car for 5 days and hotel accommodation for 2 for 4 nights. All meals, tips and gratuities are included. 

Trek Verve+ 2 Hybrid Electric Bike – APV $2,500. Bike is located and can be picked up in Appleton, WI. Winner is responsible for any shipping charges outside that area.
13″ MacBook Pro – APV $2,000 There is a 750 submission sale amount for this prize to be active.
iPhone 11 Pro Max – Prize APV $1,200 There is a 750 submission sale amount for this prize to be active.
Samsung S20 Ultra 5G – APV $1,250 There is a 750 submission sale amount for this prize to be active.
Apple 11″ iPad Pro 256GB  – APV $1,000 There is a 750 submission sale amount for this prize to be active.
5 X 1000 Amazon Gift Cards – APV $1,000 each gift card. There is a 750 submission sale amount for this prize to be active. Gift cards will be in the form of an electronic gift card. Winner will be emailed an Amazon digital gift card.
Weber Gas Grill – APV $750 There is a 750 submission sale amount for this prize to be active..
Oculus Quest VR Gaming Headset  – APV $700 There is a 750 submission sale amount for this prize to be active..
Callaway Golf Clubs – APV $500 There is a 750 submission sale amount for this prize to be active.
5 X 500 Amazon Gift Cards – APV $500 each gift card. There is a 750 submission sale amount for this prize to be active. Gift cards will be in the form of an electronic gift card. Winner will be emailed an Amazon digital gift card. 

NOTIFICATION OF WINNERS
Winners need not be present to win. Winner will be notified by phone and/or via email of the requisite procedure to claim the prize. Winner’s name will be posted at https://rockthedockgreenbay.org/ within 10 calendar days of the drawing. Winning Donor has up to thirty (30) calendar days of draw date to finalize the process (including payment of taxes, withholding, etc). Winning Donor’s name and location by city or town and photo may be printed in selected media. If the selected winning Donor is unable to be contacted or located after a period of one (1) month from the draw date, the prize will be donated to CHHC. Three (3) attempts will be made to contact the winning Donor via email and phone as provided.

VOID WHERE PROHIBITED OR RESTRICTED BY LAW
This Contest is conducted in accordance with gaming laws enacted and enforced by the Law of the state of Wisconsin and void in areas prohibited or restricted by law.
Contest promoters and/or CHHC are not responsible for lost or stolen submissions. Contest submissions will be disqualified, at the sole discretion of CHHC, if attempts are made to disrupt the Contest or circumvent the terms and conditions of these Official Rules. If the above occurs, CHHC reserves the right to remedy any such action, disruptions, or circumvention in a manner, determined solely by CHHC that is fair and equitable to Contest participants.

CHHC, its members, officers, directors, agents and representatives and their parent organizations, affiliates, subsidiaries, and advertising, promotion and legal advisors are not responsible for and shall not be liable for: (i) late, lost, delayed, damaged, misdirected, incomplete, illegible, or unintelligible submissions; (ii) failed, returned, or misdirected notifications based on inaccurate information provided by the winner; (iii) telephone, electronic, hardware or software program, network, Internet, or computer malfunctions, failures, or difficulties resulting from participation in this Contest of otherwise; (iv) errors in transmission; (v) any condition caused by events beyond the control of CHHC that may cause the Contest to be disrupted or corrupted (vi) any injuries, losses, or damaged of any kind caused by a prize or resulting from acceptance, possession, or use of a prize, or from participation in the Contest; or (vii) any printing of typographical errors in any materials associated with the Contest.

WAIVE LIMITATIONS OF LIABILITY
Donors release CHHC, and its respective board, committee members, officers, volunteers, employees and representative agents from any and all liability with respect to participation in the promotion and possession of the prize. Any liability of CHHC shall be limited to the submission price paid. In the case of a dispute, CHHC’s decision shall be final. Donor agrees to allow CHHC to use his/her name, and image without any compensation for any promotional activity related to this or any future Contest. Any and all addresses and contact information of the purchaser will not be released to the Public.
